Why One-Size-Fits-All Innovation Doesn’t Work in the NHS

By Zohaib Paracha, CEO, Cognito Consultants

As discussions around AI, innovation, and system transformation continue at NHS ConfedExpo, there’s a renewed focus on shared learning and best practice — and rightly so. The NHS is a complex, interdependent system, and learning from one another is essential.

But at Cognito Consultants, our experience has shown something vital:

> What works in one organisation won’t always work in another — especially when it comes to change and innovation.

We’ve worked with a range of NHS and care organisations, and while the principles of good governance and operational excellence are universal, the conditions in which they’re applied vary significantly.

And here’s the truth we keep coming back to:

Not All Frameworks Are Created Equal

A key hypothesis at the heart of our work is this:

> If a framework or system is already broken or dysfunctional, trying to improve it will be harder, slower, and less resilient — even if the intention and resources are there.

In contrast, a framework that is intact but underperforming has far greater potential for successful, sustainable improvement.

Let’s break that down.

An intact framework — even if it’s inefficient — still has structure, accountability, and operational integrity. With the right support, these systems can be optimised, adapted, and strengthened.

A broken or dysfunctional framework, however, lacks those foundational qualities. Attempting to layer innovation or change on top of it requires significant groundwork, redesign, retraining, and reinforcement — both pre- and post-implementation.

This distinction matters deeply when planning the rollout of new technology, AI tools, or transformation initiatives.

Implementation Has to Be Contextual

Shared learning is important — but copying and pasting solutions from one organisation to another doesn’t guarantee results.

Cultural context, workforce capability, leadership stability, digital maturity, and governance readiness all influence the success (or failure) of implementation. That’s why our approach is always tailored, context-aware, and data-informed.

At Cognito Consultants, we help organisations to:

✅ Assess the resilience and readiness of their existing frameworks before introducing change

✅ Adapt strategies to local context and capability, not just replicate external models

✅ Stabilise broken frameworks before layering in innovation

✅ Build robust governance that can flex and evolve without losing control

✅ Implement solutions that are not just theoretically sound, but practically deliverable

Innovation Alone Isn’t Enough — Readiness Is Key

If there’s one message we want to amplify at ConfedExpo and beyond, it’s this:

Successful transformation isn’t about copying what worked elsewhere. It’s about understanding what will work here — in this system, with this team, under these conditions.

At Cognito Consultants, we help clients do just that — blending strategic thinking with operational pragmatism to deliver change that sticks.

If your organisation is ready to move from ideas to impact — let’s have that conversation.
